Download the Student Application and complete the form and sign/initial both pages where indicated. Mail the form with Application Fee to: PO Box 986, Dacula, GA 30019. See link below or see our Resources page for more DCA forms and links.
After submitting the application, please mail the following materials to the above address:
- Transcript of academic curriculum with grades (required for students applying for academic classes)
- Attendance and discipline records from current school (not applicable to home school students).
- Recent standardized testing results
- Three Letters of Recommendation from non-family members (required for all new applicants). Print three forms after typing your student’s name at the top.
Please request that the person completing the form, mail it directly to the address on the form.
